Take part in South East London Maternity and Neonatal System's Perinatal Pelvic Health Service User Survey
Southeast London (SEL) Maternity and Neonatal system want to ensure that all users of midwifery services in SE London have the right access to education, support and information with regard to pelvic health, during and after pregnancy.
It is appreciated that this can be a very sensitive and private topic of conversation, and SEL Maternity and Neonatal system would like to reassure you that all the information submitted in this survey is anonymous and will be used in the strictest of confidence.
Any information you feel comfortable in providing will be used for the purposes of improving the level of service and support they can offer all women with pelvic health complications.
SEL Maternity and Neonatal system would like to hear the views of anyone who has given birth in the last 5 years and those who are currently pregnant.
